Register for Virtual Advance Care Planning Workshop

We can’t plan for everything, but we can talk about what is most important — in your life and in your health care — with those who matter most. In honor of National Healthcare Decisions Day, Hospice of the Western Reserve is offering a free one-hour virtual workshop, “Advanced Care Planning: Courage in Conversation.” This resource will help you start the conversation with your loved ones about advance care planning.
The informative workshops are available on the following dates:
  • Wednesday, April 14 at 6 p.m.
  • Friday, April 16 at 10 a.m.
  • Friday, April 16 at 2 p.m.

This presentation will take a step-by-step approach to help you start a conversation (and keep talking) so you can have a say in your healthcare — today and tomorrow. It will provide the facts and resources you need to create a care plan that’s right for you, a plan that will be available when the need arises.
We will also share our Courage in Conversation guide, which includes helpful tips for having conversations with your family, worksheets to help evaluate your personal values and goals and the Health Care Power of Attorney and Living Will forms. The free booklets can be downloaded at hospicewr.org/decisions.
